Fee: $300.00, due with entry form.
All Teams Must:
PLAYUSF SANCTIONED

BRING USF ROSTER (Provided)
PROVIDE PROOF OF INSURANCE
PROVIDE BIRTH CERTIFICATE (Upon Request)
